How do you submit a complaint?

Although we do our best to provide the best possible care, complaints may still arise. Would you like to submit a complaint about the Hollands Midden Regional Ambulance Service? Please fill out the complaints form below. In the complaints form you can fill out (personally identifiable) information in a secure way. It is also possible to submit your complaint by standard email to, in this case the information in the email is not sent in a secure way.

You can also submit your complaint in writing by sending a letter to the Hollands Midden Regional Ambulance Service complaints officer. Please include as much information as possible in your letter, using the form below as an example. This helps us to respond more quickly to your complaint
